The Providers and Staff of AV Dermatology would like to welcome you to our practice. We are looking forward to meeting you at your upcoming appointment. The following information will assist you with the registration process. In order to better facilitate information for our practice and insurance companies, please complete the appropriate forms and bring them to your appointment. If completing these forms prior to your appointment, please arive 15 minutes prior to your appointment for registration. If you prefer to fill the forms out at the office, please arrive 30 minutes prior to the appointment start time.
The Patient Forms can be downloaded to your computer and completed at your convenience. By completing these forms ahead of time, you will save a significant amount of time during your visit. You can also fax them to one of our locations.
Please be prepared to provide this information to our office along with your current medical insurance card and your photo identification. If you have a specialist co-pay, we will collect that at time of service.

Insurance and Billing
We accept most traditional insurance plans. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own dermatologist.
